We’ve launched a new, improved version of our application and data capture forms. The forms are now available and you should start using them straight away.

You can order the new forms in the normal way. The version number to look out for is 01/10 – this is clearly visible on the front page. You can also download them here:

Why have you made changes?

We’ve listened to you – your feedback is important to us and we’ve taken this into account. We’ve changed our forms to:

  • make them clearer
  • make the questions more specific
  • make them easier to complete
  • reduce the number of questions asked
  • reduce their length

What’s changed?

  • We’ve reduced the number of health questions asked so the form will take less time to complete.
  • The personal and health questions are clearer, so it’s easier for your clients to give us the information we need. This will help to reduce the number of disputed claims due to non-disclosure.
  • We’ve grouped related questions together to make the form easier to complete.
  • We’ve reduced the number of questions in our online application so it takes less time to complete.
  • We’ve updated our ‘Important notes’ to make them clearer for you and your clients.
  • We’ve improved our non-disclosure warnings to make them clearer and easier to understand.
  • For our paper application forms, we’ve put the benefits into table format so it’s easier to see at a glance what benefits are available.

Will you accept older versions of the application forms?

Yes, we’ll accept older versions until 23 March 2010. You can also apply online with an old version data capture form until 23 March 2010.

After 23 March 2010, you won’t be able to apply online with an old version data capture form and we won’t accept old version paper applications.

FAQs

Are the forms shorter/are there fewer questions?
Yes, both the personal application form and the data capture form have been reduced by four pages. There are nine fewer personal and health questions in the new forms.

Why won’t you accept the older versions after 23 March 2010?
The questions are different in the new forms and we want to move to the new improved process as soon as possible. We appreciate that you may need time to submit older versions of the forms that you’ve already completed so we’re operating a pipeline for two months.

I’ve started an online application using an old version DCF but I can’t finish it before 23 March 2010 – will you still accept it?
No, we can only accept online applications using an old version data capture form that have been fully submitted by 23 March 2010. Incomplete applications will be deleted from the system on this date. In this instance, you’d need to reapply using a new version data capture form because the questions have changed and you won’t be able to answer the new questions using the old form.

I’ve completed an old version data capture form but can’t get online to submit it before 23 March 2010 – can I post it to you?
No, we can’t accept a data capture form as an application form at all. You’d have to complete a paper application form if you want to apply by post. If the paper application form won’t reach us by 23 March 2010, then you’d have to complete a new version paper application form.

Where can I get copies of the new data capture form and the application forms?
All the new forms are now available to download and order from the online literature library.
